Please update us, as soon as possible, when you change your landline or mobile telephone number. This is extremely important that we have your correct details, as on occasions we may need to contact you regarding change of consultation, medication, referrals or results of investigations.
If you move out of the practice area, you will need to move to a new practice in your new locality. If you are unsure please ask at reception and they will advise you.
